submit a tattoo appointment request form

please read the “Booking Policies” tab before submitting a form. requests that don’t follow my policies will be ignored. policies state what I do and do not tattoo. you must be 18 years or older with a valid ID to get tattooed at Lunar Studios Art and Tattoo. please allow ample time (up to 10 business days) for me to respond to your request. photo inspirations for your tattoos can be sent once I respond to your request. thank you :)

*if you are wanting your appointment to be at the same time as another person (matching tattoos or different tattoos) only ONE of you needs to submit a form, just specify what each person is getting in the description

*please take the time to make sure your email is spelled correctly

*please don’t send in any requests with “I don’t know” as the description, I need at least a base idea to go off of
