submit a tattoo appointment request form

please read the “Booking Policies” tab before submitting a form. requests that don’t follow my policies will be ignored. you must be 18 years or older with a valid ID to get tattooed at Lunar Studios Art and Tattoo. please allow ample time (2-5 days) for me to respond to your request. photo inspirations for your tattoos can be sent once I respond to your request. thank you :)

*if you are wanting your appointment to be at the same time as another person (matching tattoos or different tattoos) only ONE of you needs to submit a form, just specify what each person is getting in the description

*please take the time to make sure your email is spelled correctly
